I don't often fill out these, but I thought I'd do this one.
Keep in mind I'm an eclectic hard-polytheist - just FYI, I don't invoke my matron and patron together. (Honestly, I don't think they trust one another. But then again, I can't really blame them.) While I hold the Lord and Lady highest, I don't count them as "patrons" as such... though I debated it, I have decided against including them in the matron-patron section.

Other Gods and Goddesses I work with: Quite a few from the Norse and Greek pantheons. I don't want to leave anyone out, but I probably will.... Odin, Sigyn, Sif, Thor, Eir, Frigga, Hecate, Hephaestos, Aphrodite, Ares, Hades, Persephone, Hermes, Artemis (for some reason, I've been working with her more often of late, though I used to rarely at best).
Animal Totems: Wolf, chiefly, though Golden Eagle has taken considerable interest in me of late, and occasionally I meet Badger and Snake helps me with healing (though which snake I couldn't tell you).
Herbs and Flowers I feel drawn to: white sage
Stones I feel drawn to: I have a stibnite that demanded that I buy it on no uncertain terms. Moonstone, flourite, amethyst, diamond
My Metal(s) Of Choice: silver, white gold, pewter
Trees that I feel drawn to: willow, pohutukawa, gum
Astro stats: Aquarius, Fire tiger
Favorite Sabbat: Samhain, followed closely by Beltane
Favorite Season: To be honest, I really do love them all in their own way. I think my favourite time of year is late autumn-early winter, when there are still red leaves on the trees, but there's a chill in the air and some early frosts.

Mythic Creatures I feel drawn to: Dragon, gryphon
Spells that come easy to me are: I rarely do "spells". What is meant by this question? Purpose of spell, or manner of spell? Knot spells are my favourite: simple, quick, easy. They make the most sense to me... I suppose they come the most easily to me.
Types of divinations/witchy services you do: I don't do "services". I'll read tarot for a friend if they ask, but they rarely ask. I wouldn't read for a stranger. In most cases, I feel people should do things themselves, if at all possible. I'll talk to a resident ghost, if I notice they're there, and if the living resident wants me to I'll "translate", but I'd never ever tell them to leave. In my opinion they have as much right to be there as the living... and I may have watched Beetlejuice too much as a child, but I'm VERY against exorcism. "It's like death for the dead."
Favorite Ritual Tool: Athame, hands down. Maybe it's the link to air (many tarot sets align swords with air, while others align it with fire. I can see reasons for either and have no real preference either way), maybe it's because I like blades, daggers, knives and swords (and maces, and morningstars, and whips...).
Type of Witchcraft you practice: Eclectic Pagan. "Witchcraft"-wise, I'm not very Wiccan... I prefer my spells very simple. But, you know, whatever works... I'm particularly interested in Nordic witchcraft at the moment, because I know very little about it. Apparently Thorsson has written a particularly good book on the subject, I must go and buy it.
